Incident Report Form

Records of any incident, accident or near-miss are kept on the form in our forms repository. Wherever possible we use the standard MSQ. form so as to avoid duplication., and a review of any incident or accident shall be conducted to prevent repetition.  In the case of a serious incident the Principal shall contact RYA./RA at the earliest opportunity and provide details.

In the case of a major Incident (such as a fatality) the Critical Incident Policy will be followed.
